While the Nogaye Thiam case emerged against a backdrop of family tensions and suspicions of assault, a cardiologist has highlighted a potentially crucial clinical element: six of the deceased's brothers reportedly died suddenly before her. For cardiologist Oumou Kébé, this pattern is not a mere news item, but a major medical signal pointing to a possible sudden death of familial origin.
“When tragedy repeats itself in a family, one must consider hereditary sudden death,” says Dr. Kébé. She emphasizes the fundamental distinction between rumor and scientific explanation: “When tragedy repeats itself in a family, medicine reminds us of a simple truth: certain hereditary heart diseases cause sudden deaths, completely independent of any violence,” she explains.
The cardiologist points out that these situations often create the illusion of a criminal act, especially in a conflictual environment, because death is sudden, silent, and sometimes "triggered by stress, fear, an argument or a strong emotion."
"When you discover a series of brutal deaths in the same lineage, it's no longer the argument that kills: it's the genes," she adds, insisting that the genetic disease can kill "sometimes at the slightest strong emotion."
Dr. Kébé lists several familial heart conditions that can lead to sudden deaths, often without any warning signs.
She discusses familial cardiomyopathies, including hypertrophic cardiomyopathy (the number 1 cause of sudden hereditary death), arrhythmogenic cardiomyopathy, and familial dilated cardiomyopathy.
The cardiologist also mentions electrical heart diseases (channelopathies): "The heart appears normal, but its electrical system is unstable: Long QT syndrome, Brugada syndrome, familial ventricular tachycardias," she says. In these families, the same phrase is often repeated: "He fell... he passed away... he was sleeping..."
For the specialist, the challenge now is to protect the surviving family members: "When several members of the same family die in the same way, medicine doesn't speak of crime, but of sudden hereditary death... To save the survivors, family screening is necessary: ECG, ultrasound, Holter monitor, MRI, genetics."
She emphasizes the urgent nature of the alert: "This is not a tragedy that should be silenced: it is a family alert that can save lives."
Family screening is an international recommendation and should be offered to all immediate family members, even those without symptoms. Recommended tests include ECG, echocardiography, 24–72 hour Holter ECG monitoring, cardiac MRI, and genetic testing.
If the disease is confirmed, solutions exist to prevent sudden death (antiarrhythmic drugs, management of triggering factors, implantable defibrillator (ICD) for high-risk cases, and regular follow-up).

On avait parlé il y a quelques années d'un projet de fichier patient unique consultable dans tous les hopitaux, il serait bien qu"il soit mis en place au plus vite pour que les services hospitaliers, lorsque les individus d'une même famille décèdent de crises similaires, puissent directement faire le lien et elles même prendre l'initiative d'analyser les membres de la famille encore vivants.
